Job Description:
Statement of Purpose:

The Senior Admissions LPN Coordinator is responsible for completing all intake/referral functions to assure efficient admissions and will serve as a resource to other Coordinators on medical issues in Access Center RN Supervisor absence.

Primary tasks, duties, and responsibilities:

1.Promotes and practices the Hospice mission and values and follows policies and procedures of Tidewell Hospice.
2.Functions as an effective team member who uses communication and leadership strategies, time management, effective problem solving and an awareness of ethical and cultural issues while engaged in the referral and admission process for patients and families.
3.Contacts physician to obtain approval for hospice services; verifies patient prognosis; obtains information to support patient’s appropriateness for care.
4.Works closely with admission team members to respond to requests for information about Tidewell programs and services.
5.“Triages” calls to admission department and serves as resource on medical issues in supervisor’s absence.
6.Develops positive working relationships with Tidewell referral sources.
7.Develops, coordinates, and implements daily plans for patient appointments and staff responsible for admission in all settings.
8.Identifies and communicates knowledge of high-risk or problematic admissions/situations to Director of Access Center.
9.Coordinates follow-up activity on all outstanding prospects.
10.Collaborates effectively with admission resource physicans, other Tidewell medical staff, and community physicians regarding admission criteria and open access authorization.
11.Monitors complex patient situations and ensures that admission RNs and HCCs anticipate patient family needs to address or communicate needs to the team IDT responsible for patient’s care.
12.Ensures regulatory and organizational compliance related to admission policies and procedures.
13.Promotes best practices for optimal conversion rate, productivity, and utilization of resources.
14.May visit referred patients to present program guidelines and obtain signed consents.
15.Tracks, compiles and distributes monthly statistical reports.
16.Communicates marketing and educational needs of referral sources to Access Center Supervisor.
17.Maintains the confidentiality of patients, family, and colleagues and reacts to sensitive situations in a professional manner.

Physical Requirements:

While performing the duties of this job, the colleague is required to sit and stand for long periods at a time; type using a computer keyboard; visually read information off of a computer monitor; orally communicate information over the telephone and in group settings; hear; and lift up to 20lbs. Specific vision abilities required by the job includes close vision and color vision.

Educational/Professional:

1.Clear and active LPN licensure required.
2.Associates’ degree in a related area preferred.
3.An equivalent combination of professional experience and education may be considered as meeting the Education/Professional requirements.

Knowledge, Skills and Abilities Required:

1.Possesses the ability to explain and communicate in person or over the telephone, as appropriate to all health care providers about the continuum of care including basic eligibility requirements for home health care.
2.Interpersonal skills with demonstrated success building teamwork and collaboration with others.
3.Organizational skills with a drive for results; ability to multi-task and work under pressure.
4.Leadership skills with the ability to prioritize, troubleshoot, solve problems and work independently.
5.Ability to maintain complete confidentiality.

7.Proficient in Word, Excel, and Outlook and proficiency in Solutions within 90 days of hire.
8.Flexible; open to new ideas and learning new skills.

11.Time management and organizational skills required.
12.Ability to work flexible hours, including weekends and holidays.
13.Possess a valid Florida driver’s license and current auto insurance.
