Job Description:
Statement of Purpose:

Works under the direction of the Clinical Director for Triage and Continuous Care (CC) and is responsible for monitoring the quality of patient care administered and supporting clinical staff during the weekend and evening hours. Ensures the appropriate flow and coordination of patient visits and serves as clinical resource.
Primary tasks, duties, and responsibilities:

1.Promotes and practices the Hospice mission and core values, and follows all policies and procedures of Tidewell Hospice.
2.Provides direct oversight and support to the Triage and CC teams during evening/weekend hours.
3.Assists in making recommendations for employment, supports the development of staff, collaborates with CD regarding performance management and provides feedback to staff as appropriate.
4.Builds a relationship of trust within the staff that is supervised and understands the work and processes being supervised. Assigns priorities and deadlines and provides ongoing coaching.
5.Serves as a clinical expert and management resource to the weekend/evening clinical teams.
6.Monitors and maintains on-call coverage, including staff/patient ratios to ensure timely response and high-quality care is being provided to patient/family needs after hours and weekends.
7.Ensures all patient documentation is complete, accurate and timely and ensures appropriate utilization of Allscripts EMR through ongoing chart audits.
8.Conducts patient/family visits to monitor and evaluate individual colleague clinical performance to include care planning, interventions and outcomes.
9.Identifies Performance Improvement opportunities in patient care, develops and monitors trends and patterns in medical records for accuracy/completion, and reports these findings to the Clinical Director.
10.Cross-trains, to the degree necessary, with clinical directors from all service lines to allow for on-call weekend/evening coverage.

Secondary Responsibilities:

Every incidental duty connected with the Clinical Services Resource Manager position cannot be specified in the job description and the colleague, at the discretion of the Clinical Director of Continuous Care / Triage, may be required to perform duties that are not included in this job description.

Physical Requirements:

1.Sitting, standing or driving for long periods of time.
2.Requires ability to orally communicate information over the telephone, in person and hear responses
3.Required to lift/push/pull up to 25 pounds independently and utilize proper techniques / assistance to lift greater weights, including the human body.
4.Visual acuity (including near/far vision) color discrimination and depth perception to read information off a computer monitor and paper documents, perform patient observation and drive throughout service area during daylight and night time hours
5.Manual dexterity and fine motor skills to type using a computer keyboard, communicate in writing and administer patient care services.


Educational/Professional:

1.Must possess current Florida licensure as RN; BSN preferred.
2.Healthcare management experience preferred.
3.Recent clinical experience required. Minimum (1) year experience as a hospice RN strongly preferred. Previous long term care, GIP care or home care work experience a plus.
4.An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.

Knowledge, Skills and Abilities Required:

1.Demonstrated ability to supervise and direct clinical and administrative personnel, serving as a role model and mentor to staff. Self-starter with minimal need of supervision.
2.Excellent written, verbal and interpersonal communication skills.
3.Demonstrated working knowledge of computers, Microsoft Office and clinical information systems. Proficiency with Allscripts EMR within 90 days of start date. Keyboarding skills 30-35 wpm.
4.Exceptional clinical, observation, assessment and problem-solving skills. Considers alternatives and applies reasoning and judgment for appropriate course of action.
5.Mental alertness and ability to work under pressure; detail and priority orientated; ability to concentrate and use analytical skills while multi-tasking.
6.Working knowledge of Hospice philosophy, federal and state regulations, CHAP standards and conditions of participation, Medicare reimbursement and internal policies and procedures.
7.Valid State of Florida Driver’s License and current automobile insurance and ability to provide own reliable transportation and travel within the four county service areas as needed.

Machines/Equipment/Toos Used:

All general office equipment including personal computer, telephone, fax/copier/scanner. Microsoft Office Suite, Clinical Information Systems, scheduling software, Allscripts EMR and clinical observation tools.
