Job Description:
Statement of Purpose:

The purpose of the Team Coordinator position is to enhance the quality of the Hospice Program by coordinating a variety of general office activities and clinical team support.

Primary tasks, duties, and responsibilities:

1.Promotes and practices the Tidewell Hospice mission and core values and follows policies and procedures of Tidewell Hospice in dealing with patients/families, community members, and internal staff.
2.Ensures and maintains processes according to policies and procedures.
3.Ensures IDT meetings are planned and organized with an agenda, invitations, back-up data.
4.Ensures accuracy of patient movement in electronic medical record.
5.Ensures patient supplies are available as needed.



Educational/Professional:

1.One year certificate from college or technical school in a related field, or three to six months job-related experience and/or training; or equivalent combination of education and experience.
2.Attends one course of continuing education per year in addition to annual reviews per education department.
